What needs to be on the invoice?

Shop Info (shop name, address, and phone number) as it is on Clark County's vendor ID PO number.
Invoice number. (can not be repeated)
Customer Information (name, address, phone number)
Voucher Number (Example SF10001)
Details on Invoice. Items & Price
- Smog test (Fail and Pass price), Diagnostic(properly explain the reason for failure, testing procedures, and needed repairs), Repairs performed, Parts (part number and price)
- Smog Test $50.00 (2) (If you are using original smog failure, which the customer pay for, only one smog check fee is allowed)
- Diagnostic $150.00
- Monitor drive Cycle $60.00 (1996 & Newer only)
- Repair Labor (breakdown by job hours), and add Total labor price
Vehicle info, VIN, Year, Make, Model & License Plate.
Customer approval signature, Customer pay amount(0) if less than $975.00 with signature, Program amount due
